Birth & Death Certificates (Vital Statistics)

What Services are available?

Certified copies of birth and death certificates are available through the Vital Statistics offices located in Bartow and Lakeland.

Birth certificates are available for anyone born in the state of Florida from 1917 to the present.
Cost: $15.00          Additional Copies: $5.00 each

Death certificates are only available for a person who died in Polk County within the last 10 years. 
Cost: $10.00          Additional Copies: $10.00 each

Ordering Vital Records Online

Vital record certificates can be ordered online using a credit card through VitalChek.  VitalChek charges an additional fee for providing this service.

Please note: Polk County Health Department does not accept credit cards for online orders. Applications for birth and death certificates are available by clicking on the proper link below.

Print and complete the application and mail to the Vital Statistics office in Bartow.  You may also hand deliver it to the Vital Statistics office located in Bartow or Lakeland.

Who is eligible?

Birth certificates can be issued to the following people:

  • Persons 18 years of age or older
  • Parents or legal guardians

Death certificates without cause of death listed can be issued to anyone 18 years of age or older.

Death certificates with cause of death listed can be issued to immediate family members (spouse, parent, child, or sibling) 18 years of age or older.

To obtain a birth or death certificate, you must present photo identification.
